I think, that is only the initial letter....
normally they will send you a letter once they have received your application....
Ýou got to wait another 2 more weeks before receiving another letter from LTA, inside, there will be another letter for Medical Checkup.
Bring that letter to the nearest Polyclinic. Xrays is also needed. The final cost should be around $44.70.. . (that was the amount I paid in June 2011, when I apply for PHVL).

Yes I have Singapore Driving License. Issued in 1992, about 20 years ago.

Yes road manner is totally different.
On the highway, they will give way to you if they know that you are driving faster than them.
If there is no traffic light at the cross road junction, every car must stop and give way to the car that stop there first. Can't imagine that in Singapore.

We seem to have things in common. I drove in the US before too. That was like 10 yrs ago. I hired a white Chrysler coupe for a week, and drove around Arizona, and even ventured into Mexico before rushing out because that country looks scary dangerous.

Driving overseas need lots of concentration.
Many fatal accidents happened overseas (concerning Singaporeans). Well their roads are straight and long.... likely you will fall alseep.....
