Mechanical obstruction? Ya...I know it's been mentioned but I find it difficult to believe. Under the microscope the organisms are not really that numerous that mechanical obstruction is likely. You see them in the lumen and occasionally invading the tissues but they are not really that numerous. It's not like spirachaetosis where the entire surface is coated with organisms. But its really a wonder how some people can live for 30 years without realising that an empty and underdeveloped scrotal sac is not normal. Just like a man i've seen who actually thought that it was normal to have 3 balls. Turned out that his third ball was an epididymal tumour that had been growing steadily for years.

Some patients who have bad watery diarrhoea only have few organisms, so I dun think mechanical obstruction is the reason. Bottom line: I dunno the mechanism but it's not my field...
