  Explanation:-
  
  Sundown effect is a disturbance of the concsciousness whereby the patient (usually an elderly) suffer this syndrome that occurs at night and is characterized by drowsiness, confusion, ataxia, and falling as the result of being overly sedated with medications. Often enough, patient may turn aggressive and interpret the people around them as strangers, in which case, initially were his/her relatives.

  In the evening, the evening sun shining on the curtains and the furniture in the hospital ward casts long shadows, which the sick patient perceives and gets scared. Hence, the term 'Sundown', cos the culprit is the evening sun.
  
  By the way, paediatric patients may get this too, other than the geriatric folks.
