In clinical experience, books cannot be relied as the first hand reference as they are not practical, we more often rely on journals and latest research, nowadays, evidence based medicine is what we talk about...
Facts need to be supported with ample evidence prior to making a decision and conclusion. Assumptions are not allowed on clinical grounds unless the need arise...

Hence, it is important to reduce all the risk factors the patient have (assess through lab investigations I posted earlier).
Treat the high BP, treat the imbalance in glucose levels, check for the source of the clot and expedite treatment as fast as possible.
CT scanning will be in an urgent list, and the patient will be sent for one and the results are almost immediate, less than 30 minutes.
Prescribing antibiotics while waiting for the bacteria culture to be ready...
